`POST /api/v1/mcp/servers/:name/test`, Test an already-installed server.

Re-handshakes the live session for reachability, then runs the optional read-only probe to exercise real operational access (scopes, grants). The response result.live_health carries the operational verdict, so the desktop can distinguish "reachable" from "actually working".

Response shape mirrors [test_connection]: success (with live_health), oauth_required, or 400.
